A child is born

When a child is born, a lot of procedures have to be carried out, such as registering him/her in the Registry Office, or to settle the question of maternity/paternity leave. The important steps are explained below.

 

Documents supplied in the hospital

-A medical maternity form (yellow)
-A statistic form (red or blue, depending on which Autonomous Community you live in) 
-A certificate by the which you assure not having registered your child in the Registry Office (the hospital takes care of this) (white)

The hospital takes care of the filling in of information concerning the delivery. The rest of it is left to your care. 

Registering the new-born in the Registry Office

The registering takes between 24h hours and 8 days after the birth. If it is after that time, you must mention it before 30 days. 

To have your child registered, you have to come to the Registry Office with the following documents:

-A presentation signed by both parents, soliciting the registry of their child (this makes the procedure easier)

-An authorisation signed by the mother for the father, in case she cannot go to the Registry Office

-A copy of the parents’ identity card

-A certificate of the parents’ register

-The family register

-The medical maternity form (supplied by the hospital, yellow)

-A certificate by which you assure not having registered you child in the Registry (supplied by the hospital, white)

-The statistic form (supplied by the hospital, red or blue) 

In case of an unmarried couple, or if you want to have your child’s surnames interchanged, both parents will have to go to the Registry.

Maternity leave

In general, the document for maternity leave can be taken care of by a member of the family, in order to release the mother from this charge. The mother’s doctor takes care of the procedure in the Medical centre. 

The documents to be provided are:

-The medical maternity form (yellow)

-The mother’s identity card in case she cannot go to the Registry

 Certificate of the company in which the mother works

The authorisation for maternity leave is delivered almost the same way as the one for illness. The company takes care of the salary of the two first weeks, and the rest of it –up to 16 weeks- is taken care of by Social Security. It is necessary to provide a certificate of the company, showing the amount of the salary.

The documents to provide are:

-The medical maternity form (yellow)

-The mother’s identity card in case she cannot go to the Registry

-The maternity leave document of the mother’s doctor 

Certificate of the company in which the father works 

The paternity leave lasts 15 days. To obtain it, you need a certificate of your company, showing the amount of your salary.

-The father’s identity card in case he cannot go to the Registry 

Procedures concerning Social Security

You have to carry out three procedures in the Instituto Nacional de la Seguridad Social (National Institute of Social Security): the authorisation file for maternity leave, the authorisation file for paternity leave, and the affiliation of the new born.

The documents to provide for maternity leave are:

-The mother’s identity card

-The family register

-The medical maternity form (yellow)

-Certificate of the company in the which the mother works

-The document “Solicitud de maternidad, adopción o acogimiento” (Maternity, adoption or hosting file) (provided by the Social Security)

-The document “Modelo 145”, filled with the mother’s personal information (provided by the Social Security)

 

The documents to provide for paternity leave are:

-The father’s identity card

-The family register

-The medical maternity form (yellow)

-Certificate of the company in the which the father works

-The document “Solicitud de paternidad” (provided by the Social Security)

-The document “Modelo 145”, filled with the father’s personal information (provided by the Social Security)

 

The documents to provide in order to register the new-born are:

-The family register

-The Social Security card on which the child will be registered (generally the mother’s)

-The new born affiliation document as beneficiary of Social Security (provided by the Social Security)

Register in the local census

You have to register your child on the Local Census lists

 The documents to provide are the following:

-The mother’s/father’s identity card (only one person is necessary)

-The family register

-The official document duplicated (provided by the Local Census, or to download here in PDF document)
